The initial appointment, which is a 60 to 75 minute session, is scheduled following a brief conversation by phone. Should you and Dr. Rosecan agree to continue to work together following the evaluation, usual appointments are 50-minute sessions. Telephone/FaceTime/Skype appointments are billed at the same session rates indicated above. Be aware that most insurance plans do not reimburse for telephone appointments.

Compliance with your individual treatment plan is required in order to remain a patient in Dr. Rosecan’s practice. Your treatment plan includes a required frequency of visits to the office, typically at least every three months, to be seen by Dr. Rosecan.
Payment of Fees
Payment of Fees
All charges are due the day of service and payable by cash, check, or credit card (Visa, MasterCard, Discover, American Express). A 7% processing fee will be applied to credit card payments. Checks should be made payable to “Arthur S. Rosecan, M.D. PLLC.” If your payment has not been received within 30 calendar days of the date of service, then the account will be charged a $35 late fee for each 30-day period that the account is not paid. Accounts which are delinquent may be sent to collections. There will be an additional processing charge of $35 for returned checks.

Charges are due on the day of service regardless of your insurance status. Dr. Rosecan has opted out of Medicare; therefore, if you have Medicare and wish to see him, you must sign an agreement that states that you will not submit claims to Medicare for his services. Dr. Rosecan does not participate with insurance plans. If you have medical insurance other than Medicare, it is your responsibility to file all claims for reimbursement with your insurance company. Dr. Rosecan will be happy to assist you by supplying you with bills, diagnoses, and any information required by your carrier, but payment is expected from you directly. Dr. Rosecan will fill out forms with you so you know what information is being shared. He encourages you to find out from your insurance company about your rights of privacy.

If you are unable to keep an appointment, please give 24 hours advance notice (weekends and holidays excluded); otherwise you will be charged for the time that was reserved for you. You may cancel your appointment by leaving a voice mail message at (561) 529-1515. An excessively late arrival with 15 minutes or less remaining in the scheduled time-slot, will be billed as a No Show or Late Cancellation and you will not be seen, because it does not allow Dr. Rosecan adequate time to assess any of your needs without affecting appointments that follow.

If you are experiencing a medical or psychiatric emergency, please call 911 or go to the nearest emergency room. If you need to call Dr. Rosecan urgently, then call (561) 529-1515 and leave as detailed a message as necessary.
Pharmacy Requests
Pharmacy Requests
Dr. Rosecan’s policy is to refill prescriptions during scheduled appointments. Please bring your medication bottles and any changes in any of the medications that you are taking to the appointment. Dr. Rosecan does not refill prescription requests faxed by pharmacies. Prior authorization requests by managed pharmacy benefits companies will be handled during scheduled appointments with you.
